The Child Support Enforcement Program will assist Band members through the legal process of child support, including paternity testing, helping determine child support obligations and enforcing those obligations.

Required documents to register all NEW Income withholding orders

• Transmittal #1 and Confidential Information Form
• Income Withholding Order (current) signed and dated
• Affidavit of Arrears (notarized) or CAFS screen (notarized)
• Copy of completed Locate Employment Verification Form
• Registration of Foreign Judgment Form (MLB form)
• Copy of the Court Order (does not need to be certified)
• COLA notices (if applicable)

Required documents to AMEND Income withholding orders

• Transmittal #1 and Confidential Information Form
• Income Withholding Order (current) signed and dated
• Affidavit of Arrears (notarized) or CAFS screen (notarized) – Even if there are no arrears owed
• If there is a change in the Order – Such as the amount of Child Support owed, we would then need a copy of the Amended Order (does not need to be certified)
• COLA notices (if applicable)

Required documents to process COLA increases

• COLA Notice
• Updated Income Withholding Order

Required documents needed to stop income withholding orders

• Transmittal #2 – Stating you would like Income Withholding to stop and the reason;
Ex. Please stop Income Withholding per CP request
Ex. No longer employed
OR
• Notice of Income Withholding Termination
