SUMMARY:
The Case Manager is responsible for the planning, negotiation and management of all of the 477 Grant program participants. This requires the creation of a development plan for both Adult and Youth clients. The Case Manager will be responsible for proactive, on-going management of the case, and subsequent follow-up with the client and program resources to ensure barriers are being addressed and goals are being met, within the AanjiBimaadizing program standards and guidelines.

QUALIFICATIONS:
• Associate’s degree in vocational counseling, social work or a related field and a minimum of three (3) years of experience working directly in the Human Services field OR; Five (5) years of case management experience in a Federal Grant program is required.
• Three (3) or more years of experience in working with the Native American community, including a knowledge of and respect for Native American culture is required.
• Knowledge of interviewing and information gathering techniques is required, one (1) year of interviewing/assessment experience is preferred.
• Ability to interpret various technical documents related to income maintenance program rules.
• Proficient in Microsoft Office systems.
• Must have experience with data entry and working with database systems, two (2) years’ experience is preferred.
• Ability to maintain a professional relationship with clients is essential to be able to make fair, unbiased decisions based exclusively on eligibility.
• Skills in job counseling, employability assessments, and formulation of client improvement or development plans are preferred.
• Experience monitoring and documenting participants, knowledge of conflict resolution and motivational techniques, and problem identification and solving is preferred.
• Ability to present information effectively to individuals and groups.
• Ability to deal with highly confidential information and comply with federal data privacy laws.
• Ability to establish and maintain effective working relationships with participants, public, co-workers and outside agencies.
• Strong organizational, communication and interpersonal skills.
• Must have security clearance to MN Department of Human Services Public Assistance computer systems is required.
• Must pass an appropriate background check prior to start.
• Must have current driver’s license, reliable personal transportation, and proof of current insurance coverage.
• Must be insurable under the Mille Lacs Band Drivers Insurance policy.
• Must pass a pre-employment alcohol and drug test.
• This position is grant funded and is dependent upon the continuation of the Mille Lacs Band of Ojibwe P.L. 102-477 federal grants program.

DUTIES AND RESPONSIBILITIES:
• Establish and periodically review continued eligibility for various assistance based programs.
• Interview program applicants, explain assistance based programs and verify information needed to establish eligibility.
• Refer program participants to other programs as appropriate.
• Evaluate participant work history, job skills, abilities, training, education and other factors to determine their ability to obtain and maintain suitable employment.
• Develop a mutually agreed upon development plan for all participants and coordinate and monitor these plans to ensure all necessary services are available and accessed. Ensure that the plans are in compliance with by the participant by maintaining adequate documentation of all cases and coordinate with other agencies and/or social service organizations that have a role in the plan.
• Coordinate and monitor services that may be needed by the participant to obtain employment. Such services may include child care, transportation, job counseling and social services. Refer participant to counseling, peer group support or other services as appropriate. Update plan as appropriate.
• Verify that all job specific training received by a participant is directly related to employment and is appropriate for the local labor market and employer needs.
• Maintain files and documentation on each case to ensure compliance with state and federal requirements and to ensure due process. Maintain records necessary to support compliance or non-compliance assertions. Maintain informational releases as required by the Data Privacy Act.
• Perform case maintenance in the appropriate database or registry system.
• Assist in the collection and compilation of demographic and statistical information to evaluate the effectiveness of programs and identify gaps in services.
• Represent the Department to participants, officials, and other reservations and communities.
• Conduct public presentations of the programs as required.
• Other duties as assigned.
WORKING CONDITIONS:
• Work is almost exclusively indoors in a controlled climate area.
• Moderate stress level due to meeting program goals and working with participants.
• Little threat of personal danger or risk.
• Hours may vary depending upon departmental needs and may include evenings and weekends
• Local travel throughout the service area is required
